What is a SIMeRP program?

A Self-Insured Medical Expense Reimbursement Plan (SIMERP) is an IRS-qualified workplace program that allows businesses to provide voluntary benefits to employees at no cost to employers and with no reduction in employees' take-home pay. Using tax-advantage provisions under IRS Section 125 Cafeteria Plan and codes 105, 106, and 213(d), it operates as a supplemental medical care membership that enhances existing employer-sponsored group health insurance, with employees making a monthly pre-tax salary reduction that lowers their taxable income while receiving tax-free reimbursements for the fair market value of actual medical care and wellness services.

For employers, the financial impact is straightforward: because employers must match employees' FICA taxes, when employees pay less FICA, employers pay less as well, resulting in a substantial net savings per employee per year.
